SENTRY trial shows early survival benefit with selinexor plus ruxolitinib in myelofibrosis

Live from SOHO 2026, Dr. Bose discusses the phase 3 SENTRY trial in JAK inhibitor-naïve adults with myelofibrosis and platelet counts of at least 100 × 10⁹/L. The trial evaluated selinexor plus ruxolitinib versus placebo plus ruxolitinib.

The results supported a supplemental New Drug Application submitted to the FDA by Karyopharma Therapeutics.

Dr. Bose highlights a significant improvement in spleen volume response with selinexor plus ruxolitinib, while symptom improvement was similar between the two treatment groups. Dr. Bose also discusses an unexpected early survival benefit and how once-weekly dosing and the use of antiemetics can help manage treatment-related adverse events.

“This would make it the only phase 3 trial of a combination in myelofibrosis with a survival benefit, and that too seen this early,” Dr. Bose said. He also noted the association between spleen response and survival, calling it “a reminder that spleen is a powerful surrogate for survival.”
